The Process

Once you’ve confirmed that you meet Kentucky’s eligibility requirements, simply follow the five steps below to complete your medical cannabis card application.

Visit a Medical Cannabis Practitioner

To qualify for a medical cannabis card in Kentucky, you must be evaluated by a certified medical provider registered with the Kentucky Office of Medical Cannabis. They’ll review your medical history and confirm whether you have a qualifying condition.

Complete the State Application

You’ll need to register and apply through Kentucky’s Medical Cannabis Patient Portal, managed by the Office of Medical Cannabis.
This is where your certified provider will later upload your written recommendation to finalize your application.

To get started, check out KY Med Cards’ state application guide for step-by-step instructions.

Pay the State Application Fee

After your provider submits your certification, you’ll pay the state application fee online through the Kentucky Medical Cannabis Program Portal.
Fees are set by the state and may vary depending on the type of application—patient, caregiver, or minor patient.

KY Med Cards will guide you through this step and ensure your payment and submission are properly completed.
